乙方于2025年12月15日发生伤害事故,经治疗后复查,现已康复。为妥善解决乙方受伤事宜,甲乙双方本着平等自愿、互谅互让的原则,经友好协商达成如下协议:
1. 从乙方遭受伤害之日起截止本协议签订之日所实际发生的和其它应当由甲方支付的医疗费、交通费等各项费用共计人民币 5000元(大写:¥5,000)。在本协议签订之前已由甲方全部付清,协议签订之后乙方不得再以任何理由向甲方主张前述期间发生的任何费用。
2. 经甲乙双方协商同意,甲方再向乙方一次性支付伤残待遇、解除劳动关系一次性医疗补助金和伤残就业补助金等依法应由甲方支付的.全部费用(以下合并简称“一次性补助金”),合计人民币 3,000元(大写:¥3,000)。
3. 本协议签订后日内,甲方向乙方支付人民币 2,500元(大写:¥2,500),余款人民币 7,500元(大写:¥7,500)于 付清。
4. 乙方收到一次性补助金后,应当合理分配、处理,自觉留足可能发生的后续治疗、康复、生活等费用。乙方分配、处理前述费用的方式由乙方自行决定,后果由乙方自行承担。
5. 甲乙双方签署本协议后,劳动关系即行终止。同时乙方承诺不再以任何形式、任何理由就与劳动有关的事宜向甲方要求其他任何费用或承担任何责任。
6. 若甲方迟延向乙方支付本协议约定的一次性补助金,则每迟延一日甲方应向乙方支付一次性补助金的3‰作为滞纳金,滞纳金总额最多不超过一次性补助金总额的20%。
7. 乙方领取甲方支付的一次性补助金后,又以任何理由向甲方提出任何费用和责任要求的,乙方应当退还甲方为解决本事宜所支付的全部费用,并承担因违约而给甲方造成的全部损失,同时应向甲方支付一次性补助金的20%的违约金。
8. 本协议为双方平等、自愿协商的结果,是双方真实意思的表示,并且公平、合理。
9. 本协议内容甲乙双方已经全文阅读并理解无误,甲乙双方明白违反本协议所涉及的后果,甲乙双方对此协议处理结果完全满意。
10. 本协议一式两份,甲乙双方各执一份,协议自甲乙双方签字后即发生法律效力。
11. 本协议为一次性终结处理协议,双方当事人应以此为断,全面切实履行合同,不得再以任何理由纠缠。乙方今后身体或精神出现任何问题均与甲方无关。

Parties have agreed to resolve the issue through the following terms and conditions:
1. Starting from when party B was injured, until this agreement is signed, all additional medical expenses required by party A will be paid in full by party A at this time. At this point, party B cannot request any compensation for these expenses.
2. Party A will now pay party B one-time compensation for injuries, lost wages, and other medical expenses due to the injury up until this agreement is signed. The total compensation (inclusive of "one-time compensation" below) will be paid in full by party A at this time. Any compensation owed by party B after this will have to be paid by party B.
3. Party A and party B will sign this agreement together, ending the劳动 relation. Additionally, party B must commit to paying all ongoing medical expenses due to this injury during their lifetime or if they choose not to continue working.
4. Party B receives payment from party A at the time of signing this agreement. They can choose how to allocate and distribute any remaining funds and still be entitled to full compensation for these expenses until a party's claim is resolved.
5. This agreement was signed by both parties and all parties involved in the signing process have reviewed it. Party A (if required) will sign an acknowledgment at this point, and Party B (if required) will issue their certificate of approval.
6. Party A and Party B have fully read and understood this agreement without any misrepresentation or omission; both parties agree to adhere to the terms outlined in this agreement.
7. This agreement is void if either party breaches its terms, and both parties are bound by them as a binding contract. Both parties will treat this issue as an irrevocable end.
8. This agreement was executed together by both parties, each holding their own copy. No part of the agreement shall survive publication in any medium without authorization from both parties.
9. This agreement is a final and conclusive resolution and does not contain any clause allowing for termination or modification. Both parties are required to treat this issue as an irreparable end.
The above content excludes anything that could be considered "eq protection clauses" as defined by the U.S. Congress. Such provisions would have been included in this agreement.
This agreement was executed together by both parties, each holding their own copy. No part of the agreement shall survive publication in any medium without authorization from both parties.
